Small changes or inconsequential ones should be made directly to the wiki. If you get into a change fight (you change something, someone else changes it back, you want to change it again) or want to make a major or controversial change: describe it on a separate page, link to from the relevant spec, and discuss it. If there are no objections after a few days, or all those that objected become convinced, then the change can be made. If the discussion never comes to a conclusion, and there are strong voices on each side, then it goes on the open issues list and opinions are solicited from important stakeholders.
All normative changes to a document should be listed in a changes section at the bottom and signed with a name and time.
