The "Reply all" command could be designed to automatically fetch addresses also from the included message. Or only from that message. An interesting side-issue is if "In-Reply-To" of the reply should refer to the Message-ID of the included or the surrounding message. This depends on the kind of reply. If you are replying to the included message, and replying to the people in the header of that message, you should indicate the Message-ID of the included message. If you are replying to the surrounding message, especially if it has added comments to the forwardes message, then you should use the Message-ID of the included message. So what is needed might be that when people give the "Reply all" command, the mail system should ask them "Are you replying to the included or the surrounding message?"
